Enrolling in the Proactive Health Management Plan is easy!
You will be completing what is called a Health Risk Assessment (HRA) in order to enroll. The Health Risk Assessment is a brief but thorough HIPAA-compliant health survey that gives our health coaches the information they need to proactively manage your health.
Proactive Risk Solutions has teamed up with The American Health Data Institute (AHDI) to bring you a data-driven application to ensure that each Proactive Health Management Plan provides the best possible coverage available.
During your employer’s enrollment period, you will receive a personalized link via email that will walk you through the steps on how to open your Health Risk Assessment.
You will get the chance to enroll yourself by completing a Health Risk Assessment Self Enrollment (HRA). The HRA will arrive in an email during the first week of enrollment.
If you do not have the time to complete the HRA by yourself, you will receive a courtesy call from one of our benefit specialists (if a phone number is provided by your employer). Our specialists serve as a friendly reminder and will assist you in completing the HRA over the phone. Assisted enrollment takes place during the 1st and 2nd weeks of enrollment.
When it comes time to enact the Proactive Health Management Plan (the 1st of the following month) any employee who has neither enrolled nor opted out will be auto-enrolled. Remember, your net pay should not decrease with the program. In the rare occurrence that your net pay does decrease with the program, you will not be auto enrolled. This takes place before the first payroll run with the program (1st of the following month).
The HRA is broken down into two sections designed to help you manage your current health situation, as well as keep an eye on any possible future health scenarios.
The personal information segment of the HRA looks similar to any other assessment, questionnaire or survey—we want to know who you are! This info is used by our health coaches to remind you when benefits are available as well as to ensure you have access to all the special features included with the Proactive Health Management Plan.
The second section of the HRA is the beginning of the questionnaire. We want to learn about your current health situation. All of your answers will be strictly confidential. No answers will impact plan participation. The more accurate the answer, the more helpful it is for you!

As a member of the Proactive Health Management Plan, you will receive valuable tips, strategies and pointers to keep your priorities straight and your health on the right track.
