To confirm your diagnosis, you doctor will need to repeat one or more of these tests: AIC, fasting blood glucose, random blood glucose, or oral glucose tolerance. If your cellphone number or email address has changed, please contact us immediately so we may update your records. If there is a family member in your household that has a smartphone and/or email access and gives you permission to utilize their contact information, we can send the information to them. If you are utilizing a family member’s contact information, we must be informed of the person’s name for our records.
In order to stay compliant with the Proactive Health Management Plan, our minimum requirement is that you open at least one communication from the program each month. We will send additional benefits-related information periodically. Our goal is that you open each message about the PHMP so that you stay well informed.
